What it involves
- Agreeing with CCG on long-term condition (LTC) to be the priority for monitoring remotely in 204/15
- Identifying the ongoing test or bodily measurements needed to support stable management of the priority LTC and the means by which the patient will access the tests and report results.
- Identifying the for monitoring test results (other than face-to-face) such as video call, phone, text, email or letter and the governance arrangements for these options including data safety and confidentiality.
