Remote care monitoring DES (England)

A summary of what is involved in the remote care monitoring (preparation) DES for England

Undertaking preparatory work in 2013/14 to support the subsequent introduction in 2014/15 of remote care monitoring arrangements for patients with long term but relatively stable conditions.

From 1 April 2013 (see above).

£0.21 per registered patient (£1,478 pa for average list practice with 6,911 patients)

What it involves

  • Agreeing with CCG on long-term condition (LTC) to be the priority for monitoring remotely in 204/15
  • Identifying the ongoing test or bodily measurements needed to support stable management of the priority LTC and the means by which the patient will access the tests and report results.
  • Identifying the for monitoring test results (other than face-to-face) such as video call, phone, text, email or letter and the governance arrangements for these options including data safety and confidentiality.
