The resources form part of a new reference guide that aims to support PCNs to get the most from the new role that is being fully-funded as part of the network contract DES.

The guide includes a checklist for introducing social prescribing link workers to PCNs, advice on how to create a shared local plan for social prescribing, a framework for how social prescribing link workers should work, information about supervision and ongoing learning and advice on quality insurance.

There are also resources to help PCNs recruit a social prescribing link worker including a sample job description, person specification, sample job advert and suggested interview questions, as well as tips on what to include in a new link worker's induction.
