Limited company

Can you advise on setting up a limited company? I am a GP doing locum sessions mostly for an NHS out-of-hours provider and some private firms. I am also a part-time GP partner. I have heard that some locums set up companies to pay less tax on their earnings with their non-working spouses. The spouse works for the company providing admin help, invoicing and so on. Is this advisable from the tax point of view? What happens to my NHS pension based on my out-of-hours earnings? Do I need to be concerned about VAT? What happens to tax relief on my locum work expenses and what will my wife's tax liability be?
